Welcome and thank you for choosing Pulse Chiropractic Care.
We love new patients. We’re here to get you back to where you need to be.
Before we get started, there are just a few things to get out of the way.
Please share with us as much of your health history as possible.
Tell us how your pain or discomfort came about.
Was it a sudden onset or is this something that developed over time? Is your pain or injury work-related or is this something that happened recreationally? Does your injury impair your movement or are you still more or less able to move about freely? The more we know, the more quickly and accurately we can diagnose and develop a treatment plan specifically tailored to you.
How did you hear about us?
We love new patients and we appreciate current or former patient referrals. If you ultimately like the services we provide or feel like writing a positive review somewhere down the line, we’d greatly appreciate it.
Let’s talk briefly about insurance.
One of our core values is transparency, so we like to be upfront with all of our patients and let them know that we no longer accept insurance. Dr. Pulse’s initial consultation fee is $45.00. Patients who would like to submit their visits for reimbursement may do so depending on their insurance. We provide patients with clinical notes, diagnosis codes, and an invoice so that they can file directly to their insurance company. We also recognize that, with deductibles and co-pays, paying out of pocket may sometimes be comparable and even a less expensive alternative. Our treatments are reasonably priced in an effort to give access to many who may be seeking quality chiropractic care. Before you make a decision, we welcome a conversation.
